THE SURGERY IS NOW OPEN FOR EARLY MORNING APPOINTMENTS WITH THE PRACTICE NURSE AND THE HEALTH CARE ASSISTANT ON A MONDAY & WEDNESDAY FROM 07.10 AM. THIS IS FOR PRE-BOOKED APPOINTMENTS ONLY. THE SURGERY NO LONGER OPENS ON A SATURDAY MORNING.
WE WILL NO LONGER TAKE TELEPHONE REQUESTS FOR ANY PRESCRIPTIONS, ALL PRESCRIPTIONS CAN BE ORDERED BY USING YOUR DRUG RE-ORDER SLIP, HAND IT IN AT RECEPTION OR PUT IN THE PRESCRIPTION BOX HERE AT THE SURGERY, YOU CAN ALSO ORDER ONLINE, TO DO THIS YOU NEED TO BRING TWO FORMS OF ID INTO THE SURGERY, ONE PHOTOGRAPHIC AND ONE WITH YOUR ADDRESS ON. PLEASE BE AWARE THAT PRESCRIPTIONS COLLECTED FROM THE SURGERY WILL BE READY AFTER 3PM THE FOLLOWING WORKING DAY, IF SENT TO A PHARMACY IT IS 3 WORKING DAYS. WE CAN NO LONGER SUPPLY SAME DAY PRESCRIPTIONS DUE TO THE VOLUME AND SAFETY FOR OUR PATIENTS, URGENT PRESCRIPTIONS FOR ACUTE CONDITIONS WILL BE DEALT WITH BY THE DUTY DOCTOR.
As from Monday 8th July 2019 Dr Syam will be retiring from the Surgery has a partner after 27 years of service, he will be sadly missed by all staff & patients. He will cover some locum sessions.
Friends & Family Test
From the 1st December 2014 all GP practices are required to ask patients the Friends & Family test questions that will be monitored by NHS England on a monthly basis. The two questions are as follows:
How likely are you to recommend our GP Surgery to friends and family if they needed similar care or treatment?

Mid Yorkshire Hospitals NHS Trust now offer a free bus service carrying visitors between Dewsbury, Pontefract and Wakefield Hospitals.
Service 113 takes patients and visitors between the three hospitals, seven days a week.
This is a free service for hospital staff, out patients and visitors.
The service operates hourly between 08.00 and 20.00 Monday to Fridays and 13.00 to 20.00 Saturdays, Sundays and Bank Holidays utilising fully-accessible minibuses and leaves from outside Pontefract Hospital
From June 2014 all patients aged 75yrs and over will be given a named GP, this will not affect who they can see.
We are asking patients for their email address's so that we can mail any updates or changes within the Surgery. You can do this online via updating your details or alternatively at the Surgery.
You will now be asked the nature of your problem which will enable you to be given the best possible service & maybe direct you to alternative services which would benefit your needs (Care Navigation)
All Doctors & Minor Illness appointments are released at 8am for morning appointments and 12PM for the afternoon appointments.
You will be offered Minor illness appointments with our highly trained Practice Nurse or our Advance Nurse Practitioner for e.g coughs chest, throat, ear infections, rashes and much more ,
Telephone advice from a clinician is also available if appropriate.
