Repeat Prescription Request

If you are not registered for online services, you can use this form to request any repeat prescriptions from the Practice. Please note that medications can only be ordered seven days before they are due and will be available to collect in a minimum of three working days from your nominated pharmacy. If the medication you are requesting is not on your regular repeat prescription or something you have not had this medication in the last 6-8 months you may need to book a telephone call with a GP. In future you may wish to consider registering for our Online Services. The Online Services system remembers which medications you are on and makes requesting repeat prescriptions faster and easier.
